Current Tobacco Use and Tobacco Use History Survey
You are being asked to be a part of a survey study. Before agreeing to participate in this study, it is important that you read the following explanation of this study.
Explanation of study: Huston-Tillotson University (HT) in collaboration with the Austin/Travis County Health and Human Services Department is trying to learn about tobacco use and health attitudes, knowledge, behaviors and opinions of students, employees, and visitors on our campus in order to determine how to best help the HT campus to achieve the University’s health and wellness goals and objectives. This survey asks about tobacco use. It should take you no longer than about 10 minutes to answer all questions.
Voluntary participation: This survey study is voluntary. You do not have to participate in this survey study. You may change your mind at anytime. If you decide not to participate, there will be no penalty of any kind.
Confidentiality: All information gathered from this survey will remain confidential. A number will be assigned to your survey and entered into an electronic database by number only.
